1. Information we collect

- Contact details: name, phone number, email address, postal address if you give it.
- Health-related details you choose to share: joint or pain location, pain level, prior treatments, surgery history, and other answers on our assessment, Meta lead forms, screening forms, or calls. This is information you volunteer so we can see whether a consult may be appropriate. It is not a medical record created by this website, and it is not a diagnosis.
- Appointment details: requested or booked consult times, location, and provider (for example, Dr. Sabeen Dhand).
- Communications: SMS, call recordings or notes, voicemail, emails, and chat messages.
- Technical data: IP address, browser type, device, pages viewed, referring URL, and similar analytics or advertising cookies/pixels (including Meta) if used on our site or ads.
- Consent records: when and how you opted in to SMS or other marketing.
We do not require you to create an account to use the website.

- Meta lead ads (Facebook and Instagram). If you submit a lead form, Meta sends us the information you entered, including any optional SMS or marketing checkbox you selected.

- Screening forms and follow-up we send after you inquire.
- In-office or consult conversations once you are a patient or scheduled candidate.
3. How we use it
We use this information to:
- Respond to inquiries and run the assessment or screening process.
- Decide whether to invite you to a consult and to schedule or confirm visits.
- Text or call you about next steps when you have consented, including screening-form links and consult details.
- Operate, secure, and improve the website and our advertising.
- Comply with law, defend our rights, and prevent abuse.
We do not sell your personal information for money.
4. Text messages (SMS)
If you opt in on a Meta lead form, website form, or by text, we may send SMS about your inquiry, screening, consult coordination, and related practice updates.
- Consent is optional and separate from submitting a form or becoming a patient.
- Message frequency varies. Message and data rates may apply.
- Reply STOP to opt out, HELP for help.
- Opting out of marketing texts does not cancel a scheduled medical appointment. Call us if you need to change a visit.
- We do not share your mobile number with unaffiliated third parties for their own marketing.
Carriers are not liable for delayed or undelivered messages.
5. Who we share information with
We share information only as needed to run the practice and our outreach:
- Our clinicians and staff, including Dr. Sabeen Dhand and personnel involved in your inquiry or care.
- Service providers that process data for us, such as our website host, CRM and messaging tools (including HighLevel / Lead Connector and our phone/SMS vendors), Meta (for ads and lead delivery), analytics providers, and similar vendors under contract.
- Insurance or billing partners if you move forward with care and we need to verify benefits.
- Authorities when required by law or to protect safety, or in a sale or reorganization of the practice.
We do not sell lead lists.
6. Cookies, ads, and Meta
Our site and ads may use cookies, pixels, or similar tools to measure traffic and deliver or measure Meta ads. Meta and other ad platforms have their own privacy policies. You can control cookies in your browser and ad settings in your Meta account.
7. Health information
Some information you send us (for example, pain scores or treatment history) is health-related. We treat it carefully and use it to evaluate whether a consult is appropriate and to coordinate care.
This website and our ads are not a substitute for a physician-patient relationship. Completing an assessment or lead form does not enroll you as a patient and does not create a duty to treat.
If you become a patient, HIPAA and other medical-privacy rules may apply to your clinical record in addition to this policy.
8. How long we keep information
We keep inquiry, SMS, and lead records as long as needed for follow-up, operations, legal, and compliance purposes, then delete or de-identify them when we no longer need them. Patient medical records are kept as required by California and federal law.
9. Security
We use reasonable administrative, technical, and physical safeguards. No method of transmission or storage is completely secure.
10. Your choices (California and elsewhere)
Depending on where you live, you may have the right to request access, correction, or deletion of personal information, or to opt out of certain sharing for advertising.
